New Patients

We are not accepting new patients at this time, but that may change in the near future in 2021. This website will be updated on this when we change this policy.   When you call our office, the receptionist will set you up with an appointment to see either the hygienist or a dentist depending on your immediate needs.

 If you have insurance, have name of carrier, any group numbers, id numbers, and claim address ready to give to the receptionist. On your first visit, be sure to bring:

1.) Dental insurance card

2.)  Any xrays you've had taken in the past five years. You can obtain your old xrays simply by calling your previous dentist  and requesting your "most recent bite wings and full mouth set".

3.) Our two new patient forms that need to be filled out can be found below here. Bringing these forms filled out will greatly decrease your wait time in our reception area. If you encounter problems downloading or printing-our business office can supply you with these forms.
